We’ve designed the Carer Journal Bundle to the go-to guide on how to care for, support and understand your loved one.

It’s great to show to family members who are struggling to understand the diagnosis and how to care for and relate to your child. It documents all you Informal Supports (family members and friend’s who support your child for free) + Formal Supports (the ones paid by the NDIS for the disability related supports) + Community Supports. We’ve designed the About Me Journal in a strengths based approach with guidance on all those NDIS terms that are complicated and hard to understand.

We designed it around the key NDIS FUNCTIONAL DOMAINS - so you don’t have to worry about missing something. We’ve got you covered!

We designed both books, the About Me Journal and the Daily Care Journal, to work seamlessly together. A great way to up-skill the support team (including support workers) to understand your loved one better. The Daily Care Journal is designed to document daily living and social community participation progress and challenges. This also keeps the Support Team accountable for those NDIS $$$ being spent. All whilst being best practice in maintaining communication between caregivers and the family.

It has prompts for you to write in the regulation strategies that work for your child, NDIS Goals, prompts to think about activities that help with each functional domain, special interests, documenting challenging behaviours, toileting, food/liquid intake, NDIS goal progress and much more!

  • TWO Printed Journals

    • JOURNAL ONE - About Me Journal: Record useful information regarding the individual’s likes, dislikes, care needs, regulation strategies etc.

    • JOURNAL TWO - Daily Care Journal: Contains two templates, Daily Care Entries and ABC of Behaviours Entries.

    The Daily Care Entries keep track of daily events, activities, behaviours and care needs.

    The ABC of Behaviours is a convenient way to record behaviours of concern, and unpack why they occurred.

    These journals make excellent evidence for NDIS planning meetings!

  • The journal and worksheet components are beneficial for carers and support workers to:

    • Learn and understand the individual’s personality, interests etc.

    • Communicate the day’s events/activities/behaviours/needs

    • Have a central point to refer back to for activity ideas, regulation strategies and general care needs

    It also provides carers with the evidence they need for planning meetings, to prove the level of support required (and therefore justify NDIS funding!).
