This evidence can help to justify why you may need funding for Support Workers or Respite in your NDIS plans (to avoid “carer burnout”). The Achievable Carer’s Statement Template has been designed to cleverly meet the NDIS Reasonable & Necessary criteria to assist you to get the funding you need.
THE DETAILS
By following the prompts in the template, you will address the NDIA criteria that they are looking for, so they can justify funding certain supports in a plan.
The template provides opportunity for two primary carers to detail their experience and role, and also prompts for consideration about the impact on siblings in the family.
The template presents as a questionnaire, but the final product produces a formatted letter in PDF format, ready for submission directly to the MyPlace Portal or via email.
When writing your responses to the questions, you must ensure to use full sentences and punctuation.
This template should take approximately 2-3 hours to complete. You can save and return at any time. Complete as much or a little at a time as you wish.

FAQs ABOUT OUR CARER’S STATEMENT TEMPLATE

A Carer Statement (sometimes referred to as an Impact Statement) is a written letter by someone who cares for or is involved in the therapy/treatment or everyday living with the individual living with a disability.
Anyone assisting a “participant” (one with the disability) who is not being paid should write a Carer’s Statement at each Plan Review. It is one tool (within the toolbox) that assists the NDIS to evaluate whether or not the reasonable and necessary criteria (which is part of the NDIS law) has been met for the supports that have been requested.

Yes (unless you are NDIA managed, then unfortunately not!).
If you are self-managed, you can absolutely pay for this resource. Being self-managed you have greater flexibility with where you can purchase supports from. You can use ‘Core’ funds, or you can use ‘Capacity Building - Daily Living’ funds as this is a Parent and Carer Training resource.
If you are plan-managed, you need to double check they will reimburse you before you purchase this resource. Most Plan Managers will allow it to be paid from ‘Capacity Building - Daily Living’ funds, as this is a Parent and Carer Training resource.

You should provide a Carer’s Statement to the NDIS before planning meetings. We recommend uploading at least 4 weeks before your planning meeting, so your LAC/planner has enough time to read it. A great idea is to upload it directly to the MyPlace Portal as each Planner must read items submitted here to justify the supports in the plan.
A Carer’s Statement is also beneficial as evidence when you are seeking to join the scheme, through the Access Request process.

The LAC/Planner/NDIA delegate use the information collected in the Carers Statement to justify how much care and support the “participant” should need.
A well written Carer’s Statement could potentially warrant “respite” hours being funded in a plan. It all depends on the individual’s circumstances.

Yes you can use your phone, tablet, laptop or computer to complete the template.
We designed this to be accessible to everyone. We learnt that many families do not own a computer and this is a major barrier to getting your evidence to the NDIS.
